New Patient Paperwork

Making an Appointment

Our goal is to see all patients on an appointment basis. Please call to speak with a receptionist to schedule your appointment.

Complete examinations or contact lens fittings may take an hour or two, so please allow yourself ample time for your visit. On occasion, emergencies can cause delays and we would appreciate your patience. If you are not able to wait, please advise the receptionist so that we may reschedule your appointment.

We ask you to please download and fill out all paperwork before coming to your appointment. You can find the paperwork on our website or by clicking here. If you are unable to bring your paperwork we ask that you please come 20 minutes early for your first appointment so you will have time to complete the paperwork before your appointment time.
Please bring your old glasses or over-the-counter reading glasses as well as a list of any medicines you are taking. If you wear contact lenses, bring them and any information concerning your lenses. You will also be asked to present your insurance card.

If you are unable to keep your scheduled appointment or are going to be late, please call us at (601) 981-1550 at least 24 hours before your appointment or you will be charged a $25.00 cancellation or no-show fee. This will help us serve all our patients better.

In Case of Emergency

Patients are encouraged to call if an acute problem arises or if symptoms worsen. Our staff is trained to answer many of your questions. They will relay your information and your call will be returned at the earliest opportunity.

Our answering service will forward all inbound after-hours calls to Dr. Williams or to the doctor on call; calls are returned as soon as possible.

Depending on the seriousness of your emergency, it may be wise to go immediately to your nearest hospital emergency room facility.

Fees

Charges for office visits and procedures are determined by the time spent and vary with severity or complexity. We request that payment be made at the time of your visit. This includes any co-pay, refractions, contact lens fittings, and contact lens supplies.

Payment Options

We accept cash, personal checks, care credit, VISA®, MasterCard®, and American Express®.

Our staff will be happy to file your medical insurance claim if we participate in your plan. However, you are responsible for payment of your account regardless of your insurance coverage. Please contact your insurance company for a list of participating providers.
